Do I need Self Cert (SSP)?

I'm currently off sick from work, have been since tuesday. I plan to go back on Monday. I get sick pay but I was just wondering if I need to give my employer an SSP form? If I go back on Monday, I would have been off for 4 days... also, my mum said I need a sickline, is this true?

If i remember correctly you need to self cert illnesses if you have more than 3 consecutive days off sick including scheduled days off. So you would need to fill out a self cert form and give it to your employer
