Dual monitor setup and positioning?

I have two 24" monitors in dual display (one next to the other). It works fine however I'm wondering if there were any best practice tips for the physical positioning of the monitors to make them most effective.

Is it best to sit in the middle of the two screens (turning your head to view the extreme left and right)?

Is it better to have a gap between the two monitors (if this is big enough it gives the effect of two separate displays as you can only focus completely on one or the other at a time) or should they be pushed as close together as possible?

Should the monitors be facing straight ahead or angled (the latter fills more of your viewing field)?

What is the best way of utilising the dual setup for basic document work and web surfing. Should one be used as a primary monitor (for tasks which you are focused on) with the second just holding media players and secondary applications. Should you aim for something else like having the Windows start bar at the side of the screen and multiple windows open across both screens?
 
All down to personal taste i imagine. I have 2 17's and i have them right next to each other slightly angled so theyre both kinda facing me when i sit in the middle. have the left one for the document(s) im writing on and the right screen for viewing documents im referring to or web pages i need etc. When just using internet its on the left whilst media players are on the right. watch films and play games on the left.
 
I have 2 x 24" set up angled so I can aim in the middle of them. However, 95% of the time I sit facing the left (main) screen... right is used for other minor things so I just don't focus on it anymore.
