Error report - what does this mean?

I had a BSOD just now while playing GTA V. Can someone please tell me what this error report indicates:

is this the first time its happened or have you had this appear before?

have you done any hardware tests at all like memtest?

can you list your PC specs as well, do you have any OC on the cpu\gpu?
 
Thanks for replies. Also posted this on linustechtips, where they told me to try lowering my overclock/increasing voltage. I have no ran memtest at allm but having ran other stability tests such as AIDA64 and RealBench for a couple of hours. Also uploaded my crash dump file:

My specs:

4790K @ 4.6Ghz 1.260v
Titan X @ 1350Mhz
EVGA G2 850W PSU

I've increased the voltage to 1.265v

The crash was difficult to replicate before so it will take some time to be sure this fixes things with the new voltage.
