Errors on USB Drives

I've got a issue which I've not been able to resolve so far and don't know what else to do and getting a little frustrated to be honest.

Asus P8Z68-V Gen3 (BIOS 3201)
Intel Core i7-2600K
Corsair 4x 2GB Matched Modules DDR3-1600MHz (8GB) X.M.P.
Windows 7 Professional x64

Every time I plug my USB pen drive into either the USB 2.0 or USB 3.0 ports 'The driver detected a controller error on \Device\Harddisk5\DR9' is logged, in most cases the drive does not appear in explorer and eventually it asks for a format sometimes disconnecting and reconnecting resolves the issue, if the drive is plugged into another machine it appears immediately and no error is logged. I notice that the activity light on the USB pen drives flashes as if its being accessed.

I have Patriot XT Rage 16GB, 32GB and Kingston DT R500 USB pen drives. I've had all pen drives RMA'd. If I plug in my USB card reader with my memory card or a USB enclosure with a hard drive it has no issues. I've disabled all the power saving features in Windows.

I'm running the latest Intel drives chip-set is 9.3.0.1019 and SATA controller is on 10.8.0.1003. I've performed a clean install removed defragmenting tools and AV software.

Originally had an Asus P8P67 Pro (Rev 3.0) thinking its a motherboard issue bought a new Asus P8Z68-V Gen3 motherboard. I've done a Prime95 and Memtest86+ test for minimum 12 hours which all passed. I've contact Asus and they were not very helpful.

Weird issue,I've same board with no issues even overclocked, all I can think of is have you got Asus AI suite installed?..I don't use it so try removing that.

I use optimized defaults apart from oveclocking of my CPU.
