Fallout 3 stuttering performance

Hi i have fallout 3 v1.7 am getting lots of stuttering inside buildings but not at the wasteland or vault 101 any ways to reduce the stuttering ?

My rig is a q6600 @3.0ghz,gtx260 am running game at 1920x1200 high graphics settings.
 
Ok ive got shadows and water effects aswell as vsync completely off theres still stuttering going on in most buildings quite annoying when outside runs very smooth.
 
There are many fixes for the stuttering posted all over the internet for this game, but for me NON have worked for me.

The best of it is, the stuttering is worse now that my PC is more powerful than when I first owned the game when it first came out.
 
I used to get the stuttering problems in closed areas, Vats and in the pitboy when running 2 x 38xx series ATI cards. More recently I fired this up gain with the 5850 and its been great.

The strange thing was, on my old setup I could change all of the settings to their lowest, dissable crossfire and still have poor performance. With the same board, processor and ram but just a new card its flawless now.

I guess its just one of those games that is a little sketchy on particular setups. Does it still stutter if you turn averything right down?
 
Im getting above 45fps on average inside its when you move and look to the left or the right when it stutters.Ive turned shadows off,water off,vsync off,aa and af off,changed ifpsclamp to 60 in the fallout 3.ini, tried different screen res still the same annoying stutter remains.
