I have a pair of headphones that go into a USB external sound card (soundblaster G6).
Every now and then, (maybe once a week) I would start the PC up, and although the software was recognised, windows couldn't detect the device, and the software wouldnt see it either.
Restarting wouldn't work but pulling the USB out of the back of my PC and plugging it straight back in would fix it, detected straight away and working as normal.
No big deal really, just one of those things. I decided though, as in the long term, unplugging and replugging the USB connector into the back of my motherboard would eventually cause the port to wear, so to avoid this I got a simple male to female USB extension cable, that way, I could reconnect it at the join, rather than the back of my PC, saving the wear on my motherboard port and also making it a bit easier than reaching down behind my PC.
Ever since doing this, and its been a good few months now, it hasn't had any issues and I've not needed to reconnect it.
Weird huh?
