Filling concrete holes in floor

Hi guys,

I've got a load of 'soft' patches appearing on a floor at work.

I've been asked to look at it (why they asked me I'll never know..)

anyways the floor is concrete with carpet tiles on top. When the building was refirb for us, the existing floor port where filled in and new ones drilled.

After 3 years the spots where the old floor port where have started to become soft as whatever they used as broken up.

What the best way to fix this?

I'm tempted to pull the tiles up, chisel out the soft bits and refill. but with what?

Ideally, I'd like to use some sort of fast drying adhesive instead of mixing stuff up.

Any thoughts?

here are some images of what i mean..

this one is a pre drilled hole that i need to fill. about an inch wide.

this one is where the floor boxes were filled in and it's started to fall apart. I can't take up anymore until i fill it as it'll create a bigger problem!
