Whenever I try to install new updates or new extensions for firefox they never seem to open.

The extension's window says that "[they'll] be installed when firefox restarts".

However, whenever I restart firefox the extensions aren't usable, and the extension window is just stuck with them being in that stage.

I have reinstalled firefox a few times, and reinstalled the plugins, but nothing seems to help.

I have also deleted out the program files folder and cleaned the registry between the uninstall and reinstall but nothing seems to help.

If you have a pre-loader installed, so Firefox loads up when your PC boots, you must close the preloader before the new extensions 'take'.
Having Firefox preloaded for quick start-up counts as having it open.

I tried a pre-loader, and it doesn't have much effect after the first open, as it tends to be cached by Windows anyway.

If you have a preloader there will be a small Firefox icon in the System Tray (although that now has a new name which I've already forgotten).
If you right-click this icon it should tell you if it's a preloader and you will be able to shut it down, which is in effect a total shutdown of Firefox.
However, shutting down your PC closes it anyway so new or updated extensions should take effect when you start up the PC.

If you are still having problems try the Mozilla forum at http://forums.mozillazine.org/
