First website contact form help

Hi I created my first website over christmas and im pretty pleases with it I did use a free template though just to give me a base to work with.

Anyway I was wondering if one of you fine chaps could take a look at my contact page its driving me mad. I basically followed a how to on youtube and did everything as they did but when you fill it out to send an email it just redirects to my hosts website when you click send its baffling me being new to all this.

"action" is the bit you have wrong
