Force format a SDHC card when not recognised inserted?

A PNY branded SDHC card was working fine, but for some reason I decided to format it inside the digital camera. Since then I haven't been able to read card on three different PC's and different card readers. They don't even recognise the card has been inserted.

Any command line I can use to force a format of card? Even if doubling click on drive letter PC doesn't recognise card is inserted.

Card still works, but limited to using USB cable now to get pictures off camera.
 
Did the card ever work in the card readers? Do they even support SDHC?

You could try formatting the card in a different device and see if that fixes it. But since it works fine in the camera, I'm betting that the card readers don't support SDHC.
 
The SDHC used to work in the card reader on laptop, but since using camera's own menu screen to format card it has no longer been recognised as inserted in any computer card read (tried three).

Have just tried formatting card with it inside camera, using USB cable so camera appears as removable media in Windows. Last attempt was FAT32 and 4096 allocated unit size. Also tried option "Default allocation size". Still not working when tried SDHC in card reader that worked prior.

Edit:
Tried one USB so-many-in-one card reader. Other two are integrated into laptops. Not sure if PTP/MTP, etc.
