Game making help

Hi guys

Doing a project on scratch to make a game for college. Needing to do a user testing form. What type of questions would you put on it? I have put stuff like what would you rate the game out of 10. What would you add etc etc.

I don't think you need to use something like Scratch to make a nice collage. When I was a lad I always managed to make them by cutting up my mum's magazines and a few travel brochures.
