GPU about to die?

I've been having a few issues with my PC since Sunday and I think my GPU is about to kick the bucket, but I'm not sure if that's the culprit or something else.

I have a 560Ti from Gigabyte, the SoC version. It was at stock in MSI afterburner, the only thing that was changed was the fan speed. I had Overclocked it in the past but it hasn't been modified for months.

At first I got artifacting in games, which never happened before, I had polygons stretching off of all models in-game and crashing after booting certain games after a few seconds (Where they were absolutely fine on Friday), I had to underclock my card to get it back to being stable so it didn't crash the game when it booted up (Memory down from 2kMhz - 1.7kMhz and core from 950-850). Next I was getting artifacting on certain webpages, which was http://mycolorscreen.com/. But apart from that it didn't occur too often, maybe just twice I've seen it. After the artifacting occurred I had my browser repeatedly crashing on me for no reason, it wouldn't do anything at all, just crashed every time I booted it and tried to look at any web page. Finally (Today) my GPU has just started not rendering Youtube videos, the audio on them works fine, but I can't see any video at all, it's just black (This happens in multiple browsers, not just chrome) and the webpage becomes extremely unresponsive until I click pause. Videos saved on my PC display absolutely fine, though. Since underclocking the card the games run okay, artifacting is gone, but everything else seems to be screwed instead.

I've tried re-installing drivers, restarting and all that Jazz, but restarting only provides a temporary fix, as the problems resurfaces after a few hours.

Any advice on what to do?

Might be worth stripping the cooler off the card giving it a good clean and re-applying some decent Tim. If it's under warranty it might be worth trying it in a different pc and seeing if you get the same problems.
A fresh install of windows can clear up quite a few issues and is always worth a shot. You could use an old drive to just install the basics and test the card.

could be power supply too but yea sounds more like its gpu going or overheating probably the latter as it seems to happen after a while of being on

GPU idle temp in MSI afterburner is reported at 31 degrees, and fully loaded is like mid 60's to low 70's, it's definitely not over heating as far as I can see.

I can try using my other windows install to see if the issues are still happening there, but I'm unsure if that would be the cause. I don't have another PC to test it out in, so that's all I can do with it really.

I recently had all sorts of problems with my 5850 dying even in windows and not in a game, turned out all it needed was taking out and cleaning off the thin layer of dust on it. Touch wood, it has been completely fine ever since.

Yeah I tried a different driver version. Something is very wrong with it.
When I put the clocks back to what they used to be, the games are back to being completely unstable, the only way I can play games is under clocking it heavily.

Youtube videos are still not rendering properly, just a black screen.

That's on FireFox with black screen on the youtube screen with an error message right? I have the same problem on my other PC with a 9800GTX+, but I think this is a separate problem...I believe it is something to do with the recent FireFox update and it's not working properly with the flash player. Watching youtube videos is fine for me on Chrome...you might wanna give that a try.

Nope, it's in every browser. Chrome, firefox and IE.

I restart and videos will render for a while, then randomly they will start getting laggy and dropping loads of frames, then stop rendering all together until I restart my PC again.

are you overclocking the pc ? are you sure the overclock hasn't become unstable which does happen I had to lower mine recently as that old voltage is no longer keeping it stable at 4ghz

Yea my CPU is over clocked, it is less than it originally was, it's down from 4GHz to 3.8GHz on the same voltage. I can test it and see later for sure, along with booting it in a different HDD to see if Windows is the cause

Have you tried to revert the CPU back to stock speed to see of the problem still occur?

I'll give it a try, but I don't think it will help, but I'm willing to try anything. Will give it a go after work.
The reason I don't think it will work is that after under clocking my card it seems to be working okay, YouTube seems to be working better. It will render the video after a short delay normally, so will have to see. Ill try under clocking my CPU and booting into a different HDD.

I can't believe it, this was the problem. I de-dusted my PC and the problem instantly vanished.

Thanks for the answers everyone.

I had horrible, horrible problems with my 580 about 2 months ago (purchased at launch) almost identical* to what you describe.

Cleaning out the card, reinstalling drivers and underclocking it made little difference, but did delay the issues. Problem ended up being any driver post-300. Rolling back to 296.10 fixed everything; I haven't yet tried the 304.79 beta and don't intend to.

LoL: Polygonal artifacts stretching across the entire map from champions, minions and turrets. Game would dip to 55 FPS in borderless windowed mode randomly, and stutter continuously. Full screen mode fixed the FPS but animations were still not smooth.
Crysis 2: Polygonal artifacting from any tessellated surface.
BF3: CTDs, BSODs, huge framerate dips.
Youtube/anything that uses flash: Failure to render, black/green screens, plugin crashed, BSODs.
