GTA 4 problem

Hi I have just re-installed GTA 4 the game is really jerky has anyone got any ideas as to what the problem maybe I am also running COD modern warfare 1 & 2 without any problems.
I have tried reinstalling GTA a few times which has not helped.

I can't figure out how to patch the game.
I am not running SLI anymore because one of my cards has died.
I will restart the game & check the settings.

Settings (Auto)
Aspect ratio (Auto)
Texture quality (medium)
Render quality (High)
Resource usage 437/724MB

I downloaded & installed graphics card & game updates the game is running fine now.
