There isn't a specific mod manager you should use, go to gta4mods and find a tutorial on there. The mod managers are all down the user preference. They all do the same job just some have a prettier UI.
Such a good question OP. I've been looking for something similar myself without luck. Generic Mod Manager was one hint but it looked like one heck of a process to setup.
No idea which version it is, I bought it from Direct2Drive last year, which is now Gamefly. I assume 1.0.7.0... Tried the ENB graphics thing a while ago, while some bits looked ok the cut scenes were over bright/blue tinged and the lighting was all messed up in some situations .
