Have to Load Explorer.exe Manually

Have a mate's laptop to fix and I'm stumped on how to fix this issue. After logging in the screen stays black and explorer.exe doesn't load. I have to ctrl+alt+delete every time to manually start it and after that it works fine. In safe mode it loads automatically. I've looked at fixes online and a lot of them say to change the windows shell registry key in winlogon as there are other parameters but this laptop's is the way it should be (just explorer.exe). I have done a Windows Vista startup repair but have not done an actual repair as I do not have Vista Home Premium disks, only Ultimate which as far as I know doesn't repair other windows installations. I have ran a SpyBot and Avast scan, both giving results but none of them fixing this issue. Anyone got any ideas?
 
This happened to me on a fresh install once, I never got to the bottom of it but a reinstall fixed it.

Is he running service pack 2? Might be worth a try.
 
when you checked the registry winlogon thing, did you know there are global and per user settings. did you check them both? also try creating a new user account and login with that.

with regards to a repair,the vista dvd contains all editions. it's the key you enter during setup that dictates the version you get installed. in other words, you can install home using your disk.
 
Installed SP1, made a new account and same thing again except I got greeted with a nice explorer.exe application error.. No more system restore points either to before SP1. I have a feeling this laptop is just riddled with some sort of rootkit/malware? Nothing is detecting it but it really is a state. Works fine in safe mode though..
