Whether the noise of the card is a problem really depends on the user, their particular system (their case and other components) and the environment it will be used in.

But it is certainly fair to say that under load the HD 6990 is one of the loudest modern graphics cards - tests show that in controlled circumstances it is louder than a GTX 480 (stock cooler) when under load (like playing games), and the consensus is that the GTX 480 is a rather loud card.
