I would like to get the Sony MEX-GS820BT head unit (45W RMS), but the manual says "Use speakers with 50W RMS or higher power input capacity", but my car speakers are only 45W RMS (Alpine 4" COAXIAL 2-WAY SPEAKER - SPG-10C2), will bad things happen? The speakers are very new so I don't really want to replace them yet.

You should be fine with those speakers. They are 4 Ohm, which is the impedance the unit's power output was measured at.

RMS rating being given on amps is more beneficial than it is on speakers. It's rarely given on hifi speakers, so I don't see why car speakers should be any different. For speakers; what you need to know, is the max power handling and Ohm rating; they are more important than any RMS rating.

An RMS rating on an amp is more beneficial, because it gives an indication what the amp can output as clean power before it starts to get into distortion.

Those speakers have a peak handling of 180W and are 4 Ohm, so they will easily take 45W and more from the unit.

As long as you don't push the volume too high than you should be fine.
