Help me downsize my rig

Hey Peoples

Looking at shrinking my current rig down to a mini-ITX board, but need a bit of advice on the best way to go about it. I currently have the following.

CPU: AMD Phenom II x3 720
RAM: 2x2Gb of DDR2 cant remember the make or speed
GFX: Nvidia 470GTX

So I'm not sure what the easiest route would be is there any ITX board that would accomadate my current rig, or would i be better upgrading things, Ideally i didn't want to spend too much doing it.

For a case I was looking at the Sugo SG05. I basically want the smallest case i can get my gfx card along with 1x HDD and 1 x SSD in.

Also the ITX board needs to output standard 5.1 as well as an optical output at the same time if possible
 
I'm not sure if there were any reasonable AM2+ ITX boards? AM3 ITX boards seem to be fair expensive still.
I've got the Zotac 890GX ITX for my Phenom II and think it's a great board, would be worth checking the used market I guess. Would still need to buy DDR3 but that's still reasonably priced. :)
What CPU cooler do you currently have?
 
Yea Upgrading the RAM wouldnt be too bad if i could get hold of the board. Have a thermaltake BlueOrb i think its called, had it so long I cant remember lol.
