Help - MSI GD65 vat rebate form

Hi,

I know I've got til the 31st to send my receipt and bits in but they've taken the form down from their website, I was meaning to print it out but cant now. Dont suppose anyone has a copy they downloaded which I could get a copy of please ?

I got an email today saying they had recieved my form and are processing it now. Nice touch letting us know i think.
