I completed it for you.

I wasn't too keen that there was obviously an empathises to find out what people thought about females in game. Like the question below:

How likely are you to purchase a game with sexualized women in the advertisement?

Also this question confused me a bit.

To what extend do you believe that women are well represented in traditional media?

I made comment on it in the survey. By traditional media I assume you meant TV/Movies/Music etc.
