How are you on yer difficulty settings?

Depends on the game, FPS is nearly always default (but I have noticed they are getting easier!!), strategy games will get ramped up until I've beaten them on the hardest level.
 
#2 for me, borrowing some of the reasons from #1 - I don't have to time to keep plugging away, I want to experience the main content of a game and then move on to the next one. I do need some degree of challenge though.

As a general rule I'd say if it takes me more than half a dozen times to beat a given section of a game then I start getting frustrated and look at walkthroughs.

Personally I'd say you need more granularity between options #2 and #3; you've jumped from dying once or twice to it being a real challenge and needing 14 attempts to get past a given section.

#5 is an experience I can get from multiplayer and I really don't want it in SP.

Pretty much bang on my opinion. If I want my backside handed to me I go play MP, as I suck in most games against other humans.
 
Default or the 'middle' difficulty for starters, and on subsequent playthroughs or even whilst still on the first go, raise it up, keep it the same, or (can't remember ever having to do this! /toughguy) lower it a notch - all depending on how well I've managed things.

One exception would be a sequel to a game previously completed on highest difficulty. For example, I began ME2 on Veteran...and even found that too easy so I now only play on Insanity. If it's done the same way, then I'm hoping it'll be possible to choose Insanity from the very start on ME3, else I may consider modifying the game files to do so.

And yeah, it also depends on the type of game and what changing the difficulty actually does.

Depends on the game/what the difficulty does.

Games like cod with infinite enemies etc meh stick it on normal, but games like stalker where it affects other things too and instead punishes mistakes more than just making the enemies do more dmg, or the old fallout games where it increases skill check levels so you have to specialize more i stick it up to hard.
 
Most games Im useless at, i play on easy, I started playing Vanquish last week, and on easy, it was pretty easy, as it was a good game and I was enjoying it, i bumped it up to medium difficulty, i dont like replaying bits over and over again.
