How do I do this in Pages?

...where the columns spill over pages if they need to (well the text does)

Do you want the title column on each page or just the first one? If you want it just on the first one you can create a table with 1 column to act as the title.

I've tried using tables but one cell can't spill over into a new page which is a problem.
