Not saying YOU do, but generally not running spyware/adware/ malicious software, dodgy warez, demos, trials, unknown binaries from unknown sources is the No1 way to keep a 'clean os'. Just using the OS doesn't make it go wrong In my experience. And I'm pretty harsh on my windows installs. I very rarely unisntall anything.
The only system maintainance I'll do is every so often disable some of the rubbish that gets run at startup, e.g system tray icons adobe fast load, office fast load etc.
