How often do you replace backup tapes?

Hey people simple one really,

What is the average time you change backup tapes? we have some customers still using backup tapes from 2005 and the backups are getting erratic..how often do they need to be replaced? (best practices)

When you guys mean errors do you mean hardware read/write errors or even software?
