more buttons are very useful in MMOs but only in an accessible, comfortable package.

I think this is taking it too far. You could have a third of those buttons and use Ctrl/Shift/Alt modifiers and still have plenty of actions available at mouse level.

I used to play a Protection Warrior in WoW with no less than FORTY distinct usable actions and did perfectly fine with regular mouse buttons + thumb back/forward and a second scroll wheel on a RAT 7.
