IMEI Number.. Need help.

Hey guys. Got a problem, hopefully there is a easy solution.

Basically, I got my phone insured earlier this year with Barclays, I went out fishing the other day and dropped my phone in the lake. I got it out but it will not turn on.

I got a claim form through the post today, filled it out, but I noticed on the form it also says:

"Proof of your mobiles IMEI number"

This is what I need help with. Because without this nothing can be done. The form recommends that I look on my box of my phone, problem with that is: I don't have it anymore.

Basically, I'm asking how can I get adequate proof of my IMEI number to send back with the form. It needs to be done within 21 days.

Ah balls. Maybe not then!

Edit: Apparently you csn get it from iTunes.

Open iTunes.
Open Preferences. (In Mac OS X, choose iTunes > Preferences. In Windows, choose Edit > Preferences.)
Click the Devices tab.
Position the mouse over a backup to display the serial number of the backed-up device. iPhone and cellular iPad models also display the IMEI/MEID and Cellular Data Number

Just tried that, it does work. But would they see that as enough proof?
 
Okay have sorted it.. but on the form it says:

Premises/method of transport":

I haven't filled it out, I already have the address of where the incident happened, so I'm not quite sure what they are asking?
