I have the DEFCON 2x - MSI X99A SLI Krait bundle:
Intel i7-5930K 3.50GHz @ 4.00Ghz
Motherboard: MSI X99A SLI Krait Edition (MS-7885)
Memory: Kingston 4x8GB (DDR4-2400) Quad Channel (256 bit) Frequency 1199.8 MHz
CPU Cooler Asetek 570LX 240mm High Performance CPU Cooler

This has been running for 3 months now and is very stable so I thought I would increase the CPU some more.
Has anyone taken this bundle to 4.5Ghz and if so, what settings are applied?
If not, what can I change to get there?

Yeah. I got it to 4.5 stable. I upped the vcore to 1.275 and set it to adaptive power so that it clocks down on idle. Don;t think that too much else changed aside from me reverting to XMP memory mode.

I am using a totally different cooling set up though which may influence the additional cooling potential.

Each CPU is different you should try upping the multi and using Vcore to stabilize. I would not use more than around 1.3 Vcore on that cooling.
