Do you have the phone in your trouser pocket when listening? If you do, it might be putting pressure on the jack when commuting (sitting down).

I tend to stand 90% of the time as it's a short journey.

However I have been throwing my phone in my work bag when at work and that has probably been causing pressure.

I am now carefully removing the headphones and putting them in a case.
