Intel HD4000 GPU

No, probably not, that's why I use a 730 GTX with mine (single 1440p display or single 1440p and 1 1200p) :(. Was disappointed with this too at the time when I thought it would be useful between using gaming cards. I think the HD4000 is capable of 1440p but the 1440p is just not supported - think it's down to the mobo whether it will do It or not. So two 4k and one 1440, no

I use a Gigabyte Z77 UD5H

Dual monitors yes but if your monitors starts dueling you have an entirely different problem ;).

Looking at both the CPU and the display ports your motherboard has, while it can do 2560x1440 via Displayport, it can't do 4K.
