Interactive Form / Wordpress?

Hi I'm looking for something similar to this form where you can tick boxes or enter criteria that then shows an appropriate output.. in this case which therapist is best matched

Not great with server side code yet - could you show me any examples or give an example? I know what you mean just wouldnt know where to start myself
