Please tell me this isn't right or even possible.
Doing some OCing to try and get my 3dMark score up and noticed this.

I would say nothing to worry about - it is just the twelve volt rail going slightly above 12V, this was probably under load when testing so quite normal I would say - it is within spec.

Erm...I'm more worried about the VDDC at 31 volts instead of 1.212

Your gpu would be dead if it was pulling that voltage. One of my 670's often reports its max tdp in gpu z as 395 watts or something mental, but its nowhere near really.

Sorry - did not read properly, but as others have said, most likely a glitch and nothing more as the pc or gpu would have gone up in flames by now.
