Is this AMD 6970 dying or driver issue?

Hey all, can anyone help diagnose whats wrong with this 6970?

Symptoms:
Horizontal/Vertical line flicker. Line width is tiny.
Driver crash/recovery in desktop/ingame may lead to white or black screen.
Half the screen moving to other side. See pictures in spoiler tag

Symptoms occur sporadically both in desktop or while in game.

Tried but didnt solved anything:
Reseat cooler, reapply thermal paste and pads
DDU to catalyst 15.7.1
DDU to crimson 16.2.1 legacy

System:
HIS 6970 IceQ
SuperFlower 650w golden green
Win10 64bit

Likely graphic card issue.

I'd suggest upping voltage a bit to gain some stability if you really don't want to sell buy new card though. No guarantees it might work and it might kill your card.

Never really thought about upping the voltage will give it a shot.

If its really dying I have funds ready and just waiting custom 480s. Hopefully some of them will fit a prodigy case.

335mm according to Tech Report for the Prodigy.
Sapphire Tri-X/Nitro 300 series using 3x90mm fans are ~310mm, the Red Devil is using 3x80mm fans but with slightly larger spacings so looks very close to that figure in length.
