Looking for simple RSVP script

Arite folks,

I am working on a small website for my upcoming wedding with the idea that people can use it to send me an RSVP if they will be attending my reception party.

The site is still being worked on but can be found here:

I am looking for a simple form that will sit on the left hand side and allow people to enter name, email, phone number, comments and if they will be attending ot not (radio button for this). This would then be stored in a DB and a confirmation mail sent to the user.

Ideally I would be looking for a free script but just open to suggestion for now.

If you don't want to do it manually you could try generating one with this or this (click the big red 'create a form').
