Macro to switch Audio and Video Output?

I am currently using my main PC as both an HTPC and a Games machine. I have a Philips TV in the Lounge and a Samsung SA350 on my desk. At the moment I simply swap the HDMI cable over to switch between the two. I also have the faf of running the sound over HDMI to the TV and then having to swap it over to the Xonar sound card to listen to my headphones when I game.

What I'd like ideally is a software solution that lets me swap between the configurations at the touch of an On-Screen-Button.

Config 1: Audio and Video out over HDMI to Philips TV
Config 2: Video out over DVI and audio out over the Xonar sound card output to headphones.

Can I set up a macro simply in windows to do that?
 
I have found a solution for the Graphics, but still need something for the audio.

Is there any way to associate individual programmes with different sound outputs - ie. MPC goes to the hdmi, Far Cry 3 goes to the headphones?
 
Do you use specific software for your HTPC? ie. Mediaportal/XMBC?

If you do you can specify which device to output audio to within the configuration settings (of mediaportal at least). This allows you to have audio from that program to always go to HDMI for instance and then everything else goes through the windows default, which could be your headphones.
 
I have dabbled with XMBC - I really should get into it properly but am always put off by the fiddliness of trying to get it to scrape and display TV series properly - if this can be done in XMBC that might be the kick I need to pull my finger out and get it set up!
