I think he's talking about deleting the trojan file, not windows defender....

No. When you run the Virus Scanner and pick up a virus, if the Anti-Virus cant delete it you will have to go into safe mode to delete it. As some viruses rewrite themselves to the registry etc.. Safe mode doesn't allow them to rewrite themselves...
 
Basically the rule I use is:

Start by running spyware programs in standard windows mode

1) if nothing serious is found (ie no High threats) then delete/remove using the program normally

2) if High threats are found (or threats CAN'T be removed) then boot into Save mode and repeat

Simple, yet effective :)
