Missing Image Files from WHS 2011

I'm hoping someone might be able to help with a problem i'm having. I'll try and keep this as brief as possible.

We are running WHS 2011 on an HP Microserver with 4 HDD's installed. The OS is on one drive, one drive is for data (Approx 15% full), another is for the backups and the fourth is empty. Two people access various files within various shares daily (One from a Windows 8 PC and one from a Mac running Yosemite).

We have an images folder within one of the shares that has approx 50,000 images within various sub-folders but images seem to be disappearing. I can go to a certain folder to look for a certain image to find it just isn't there. I then check several of the server backups and can't find the image there either. It's as though it never existed.

We have many images missing that we are 100% certain we created and saved to the server. The only files that seem to go missing are .jpg images.

Some other possibly relevant info:

We have quote a few files that have the same file names but are in different folders (eg. we might have a file called 001.jpg in a folder called "96dpi" and then another file called 001.jpg in a folder called "Web Upload")

Images are most often created in Photoshop or Illustrator and then saved directly to the server share one by one. Sometimes files are saved to a workstation's desktop and then copied to the server in bulk (Maybe a few hundred files at a time).

There seems to be no pattern to the missing files. Just random files from the images folder. But always .jpg's.

All HDD's are shown as healthy with no problems or bad sectors.

I have searched the internet for hours but can't find any info.

Are they actually missing, or relocated?

I was always having to sort out "missing" files at work, where someone had managed to drag & drop the file into a sub-folder.

Or, could it be that someone has a "duplicate file" finder, which is set to monitor this share?

I searched the entire data drive for the filename of one of the missing images and it returned no results so I don't think it's in the wrong place.
 
Its a windows feature that caches network folders so you can still access your files when not connected, chances are its not your issue but it does have a great habit of not syncing files properly!
 
Do you have Volume shadow copy enabled? Might be worth checking there.

Also do the programs you are using have a recent file feature? you could check there and see the location it was saved.

When files go missing my process is:
Get the filename and date
Search the whole server
Search the whole workstation
Check VSS
Check recently opened files

Also what are you using for backups?

I'm not sure about "offline files" as I can't find a setting anywhere for this.

I think shadow copies is enabled as there are lots of previous versions of the folders available in the properties window. The missing files are missing from the previous versions that I checked too.

We use the WHS backup feature for backups. All of the data is backed up to another drive in the server 3 times a day. The missing files can't be found in any of the backups either.

It's as though the files never existed but we know for certain they did.
