I'm currently running a LG 34UC98-W but i'm find a few limitations with the 21:9 format with streaming but all other applications I use seem to work well. I guess the main reason for this post is what would be best to do? Add a second gaming screen (27" 1440P 144Hz G-Sync or FreeSync) or replace the LG with two 27" screens.

Outside of the streaming / gaming the main uses for my screen are Photoshop and Premier Pro so a IPS is preferred.

Given that you've listed both gaming and Photoshop, I'd say get one screen for each task. A 144hz TN seems to be ideal for fps gaming, but there's nothing better for graphics work than a high quality IPS. I find Dell Ultrasharps are always reliable for the latter, factory calibrated to high accuracy, long working life. In fact, I don't know how long they last, I've never had one develop a fault in ~11 years of buying them for home+work :)
