Monitor randomly loses signal?

I've recently instaled a saphire x1950pro which has been running fine for about 2 weeks until yesterday when playing games the monitor would lose signal, although the pc did not shutdown it still required a reboot to get it working again.
This happened a number of times. Does anyone know if its a PSU problem, Driver issue or what?

Current PSU stats are:

450w
Output:
+12v 20A
+5v 40A

+3.3v 28A
-5v 0.5A
-12v 0.8A

Any advice will be much appreciated.
 
3.2ghz intel core 2
2gb ram
Asus M/board (cant remember the actual model num sorry)

The CPU runs at 28c - 34c so its not over heating...
+12v is running 11.93v under load...
 
That PSU would prolly be ok with that rig.

What is case / graphics card temperatures?

Have you tried a different PSU or graphics card?

I assume you have already checked in the event viewer, made sure the cables are firmly connected, graphics card is firmly seated in the slot and tried a different driver set for the graphics card?
 
No i havn't tried another graphics card.. you think im made of money?
Troubleshooting it doesn't matter if it isn't state of the card, even a PCI card might be able to tell you if it something other than the graphics card (assuming the 2.5 Watt power draw doesn't fix a PSU issue, etc) :p
