MS Word 2013 template help

Hi all,
I'm trying to create a template for our site workers to walk around site, taking pics and write snagging lists.
So, with my limited knowledge on Word i want to create a template, whereby there is a picture frame on the left of the page that you can either choose to insert a picture (from file) into or to take a pic with your built-in camera and have that inserted into the frame.
Also would it be possible to markup the picture? As in, maybe draw a circle around or highlight a part of the picture, basically scribble on it using some picture tools etc?
To the right of the page would be a text box to fill in details etc about the picture.

Long shot i guess, but any help would be appreciated.

I dont think you can directly insert a photo into word without saving it first but may be wrong.

What devices are people using? Laptop or tablet? From the sounds of it a tablet with Evernote on would be perfect for what you need.

Our guys are using ipads and there is an app called Site Audit Pro that does all this, which is what i'm trying to emulate. Why? because we're making the move from iPad to Windows tablet (for many reasons).
Evernote doesn't really do what we want; that's more of a not scrtibbling app really.
Ourds need to match a certain find of visual look to what our clients require.
