Multiple Windows Sound Outputs

Has anyone ever got Windows to output sound simultaneously to multiple outputs?

Let's say that a computer has the following physical outputs, and you've got a couple of monitors plugged in on the HDMI/DisplayPorts:
  • DisplayPort(s) on ATi/NVidia Graphics Card(s)
  • HDMI(s) on ATi/NVidia Graphics Card(s)
  • TOSLINK on Motherboard

Is there anyway to send sound to more than one of them at once?

I'm not talking about anything clever to work out which application the audio is from and which monitor that application is displayed on. I'd be happy with sending exactly the same overall 'mixed' audio to all of the ports.
 
I use traktor and asio4all and with it I can send hdmi audio out from the laptop whilst monitoring the incoming channel on my headphones.

On my pc I can do the same using the onboard realtek but only using the front and rear ports. I couldn't get it to do it from the nvidia hdmi out though.

Certain things are possible but as above, the mixer/splitter/receiver option might be best here.

Also can you not just change the default sound device each time instead, assuming you want to use differentspeakers each time?
