Can someone please explain what I need to do to get an MX record sorted.

Am I right in thinking that I need to get my ISP to 'host' the MX record for me and direct it at an internet facing IP. The on that router I forward traffic for port 25 to my exchange server

Is there anything else to it? I've seen various references to people talking about MX records and mail.mydomain.co.uk for example

Well first you set up an a record like mail.mydomain.co.uk that resolves you one of the external IPs on your router.
Then you add an mx record for the mydomain.co.uk domain with mail.mydomain.co.uk as the entry. This will tell other mail servers that mail for mydomain.co.uk must be sent to mail.mydomain.co.uk, which will then resolve you your IP address.

Apart from that you are correct.
