mysql for phpclanwebsite help

Im trying to set up phpclanwebsite from this website. However im having trouble with the mysql settings

http://www.phpclanwebsite.com/index.php

Im entering the settings as it says however im getting this error. Here is the forum I have to fill in to get it working.

http://www.sephirothpd.f2s.com/paclan/install/install.php

im getting You "entered incorrect MYSQL values. Go back and check the values" error.

Ive tryed alots of different combinations to try and get this working and rang my isp,

the info I have from my isp is as follows

database address db.sephirothpd.f2s.com
username sephirothpd
name sephirothpd
engine mysql
port 3306
max Quota 10 Mb

I then have an option to enter database tools where I have a few options

however the first thing it says is

MySQL 3.23.58 running on db.sephirothpd.f2s.com as [email protected]

if i click on show processes i get this

12410682 sephirothpd i-194-106-56-62.freedom2surf.net None Query 0 ---

if i click on databases i have one database just called sephirothpd

any help identifying the correct settings I need would be helpfull
