Need some help with a single slot card

Im looking for a single slot card that has an HDMI, DP and either another HDMI, DP or DVI like the card below.
I am struggling to find a card like the below that is in stock and has the desired outputs.
Any that I have found have a VGA output which would be unusable in this instance.
Can any of you recommend a card?

I have a TV, gaming monitor and a regular monitor.
Rather than physically switching cables i want to have the ability through a gpu and software.
