Network/signal issues following update to Cyanogen 12S

I have a Oneplus One and yesterday Cyanogen OS 12S (Android 5.0) was made available OTA. I duly installed the update but now I'm having issues with getting phone/network signal. Previously at home I would have no issues with signal but following the update I was getting zero signal (I'm on Three).
I've tried fiddling around with the APN settings: resetting the APN, getting Three APN settings from the internet, letting it select network automatically - nothing seems to work. Occasionally it will show 1 or 2 bars, but as soon as I try to make a call it won't connect and goes back to showing no signal.

However it did get some signal when I went shopping earlier today (few miles from my house), but it was still showing an exclamation mark by the signal indicator (although I did manage to make a call).

I decided to revert back to Cyanogen OS 11S which I had no signal problems with previously, but now that I've reinstalled it I'm still getting the same signal issues :(.

Is there something obvious I'm missing? Anything I can try? My phone is effectively useless now :(.
 
I have had it in the past that a rom flash (on a different device) corrupted the IMEI number on my phone. Are you able to check that your IMEI number is still the same as when you got it?
 
When I reinstalled/flashed Cyanogen OS 11S it did a factory reset, but no luck.

Checked the IMEI with what I put down for my phone insurance and they match, so seems fine.

Going to try and flash the 44S modem, see if that helps.

EDIT: I've tried flashing the 44S modem, it seems to go successfully using command prompt and fastboot but the OS version still shows up as 11.0-XNPH05Q rather than 11.0-XNPH44S under 'About phone' in the settings menu. Any ideas or good links to comprehensive flashing guides?
