Looks like Tomtom are releasing a new app, based on the same Navkit software as their latest Tomtom Go PNDs.
It's been launched initially on Android in Italy with other areas to follow (and presumably an iPhone version).
Low mileage users get free navigation including traffic. Free mileage rolls over from one month to the next. The app license is bought on a 1 month, 1 year or 3 year basis.

The exisiting Tomtom app will be gradually phased out. No word on whether there will be any free or discounted upgrades for existing users yet.
Stiff competition on Android with Google maps continually improving and Route 66 Navigate (which uses the same maps and traffic data as Tomtom) being cheaper for all but very occasional users.
