Got my site - www.eteknix.com and over where announcment is i want to put a box and a button so people can subscribe to our mailing list.

That is the mailing list there.

How would i incorporate the fields from that page into one of my widgets on my wordpress site?

All sorted now anyway. It came with my theme matey.

It's part of the design.

It's the jquery cycle plugin (you also need the innerfade plugin too).

Used it recently and its very easy to setup and looks good.
