No signal on monitor

I have bought:

XFX ATI Radeon HD 4850 "XXX Edition" 512MB GDDR3 TV-Out/Dual DVI/HDMI (PCI-Express) - Retail

Asus M4A78 AMD 770 (Socket AM3/AM2+) PCI-Express DDR2 Motherboard

OCZ ModXStream Pro 600w Silent SLI Ready ATX2 Modular Power Supply

AMD Athlon 64 X2 Dual Core 7850+ Black Edition 2.80GHz (Socket AM2+) - Retail

Samsung SpinPoint F1 500GB SATA-II 16MB Cache - OEM (HD502IJ)

Corsair Dominator 2GB (2x1GB) DDR2 XMS2 PC2-8500C5 TwinX Dual Channel (TWIN2X2048-8500C5D)

Coolermaster Elite 330 Midi Tower Case - Black (No PSU)

When i turn on the system i get no signal to the monitor at all. I have tried another monitor and cable.

There are two flashing red lights on the graphics card (D1601 and D1602 ). The led on the motherboard lights up and all the fans work including the one on the graphics card. I get no beeps (i do if i take out all ram). I got a RMA on the motherboard, but when i was getting it ready to send i noticed some scraches on it so it wouldnt have been replaced. I bought another motherboard but I am still getting the same problem.

I do not have access to another system to try the graphics card with.

Are all power connectors connected to the gpu and motherboard including the 4 or 8 pin for cpu power - sometimes gets missed ;)

HD4850 LEDs

D1601 - Over temp protection enabled

D1602 - EXT 12V fault

Yes all connectors are connected to the gpu and motherboard.

I know what the LEDS mean, but I was not sure with them flashing, when i take out 4 pin cpu power they are solid leds not flashing
 
I have stripped down the system and rebuilt it. the leds on the graphics card only flash for 3 seconds now but i still dont get any beeps or any signal to the monitor. I do get beeps if i remove the RAM.
