Nokia 6220 - Software reset to defaults?

My wife got a Nokia 6220 phone off the bay and it looks like the software is messed up.

Whats is the best way of resetting the phone to its default settings?

Or is there a firmware I could download?

This comes up with a lock code when i try to reset to factory settings?

Phone calls work but i cant send text messages?
