What is the best mode of defragmentation for performance? To consolidate free space only, or to do that and order files by access time?

I've allways used the "Name" defrag. It takes longer, but when you open an app, it flies. Boot is fast as well, because all the boot files are at the front of the disc.
 
Surely by name means alphabetic? Meaning only applications and documents beginning with early letters in the alphabet would load quicky.

Defrag by access for the win.
