I have about the same system (faster processor though) and Pci-e 6800 (so only 12 pipes) but its the 256mb version, and only 1gb of ram.
It runs fine on mine at 1280*720 with decent eye candy on. Did you do the nvidia frames to render tweak?
And small areas with lots of NPC's have always been laggier than running outside for me. Figured this out when I was running it at 1680*1050 and having too much mouse lag in buildings.

Places that are worth "taking a look at" are castles. Especially if there are display cases around a count/countess' throne, they tend to have reasonable items in there to steal.
